Tadalafil — the long-window PDE5.

Tadalafil is the long-duration PDE5 inhibitor. 24–36 hour window means spontaneity instead of timing. The daily low-dose option also doubles as treatment for benign prostatic hyperplasia (BPH) — the most efficient two-indication overlap in men's health.

What it is

Tadalafil is a PDE5 inhibitor with a substantially longer half-life than sildenafil — about 17.5 hours, versus 4 hours for sildenafil. That translates to a 24–36 hour clinical window per on-demand dose, or steady plasma concentration on daily low-dose protocols.

FDA-approved as Cialis in 2003 for ED, then again in 2011 for BPH at the 5mg daily dose. Generic tadalafil has been available since 2018. We prescribe the generic.

How it works

Same PDE5-inhibition mechanism as sildenafil — blocks the enzyme that degrades cyclic GMP in penile and prostatic vascular tissue. The longer duration comes from structural differences (rigid ring system) that resist hepatic clearance.

The BPH indication exists because PDE5 also operates in the lower urinary tract and prostatic smooth muscle. Daily low-dose tadalafil relaxes that tissue, improving urinary flow in addition to its ED effect.

Dosing and cadence

Two dosing patterns. On-demand: 10 or 20mg, 30+ minutes before activity, with a 24-hour minimum between doses. Daily low-dose: 2.5–5mg every day, which produces steady-state coverage and covers BPH symptoms if relevant.

Food sensitivity is minimal — fat content of meal doesn't meaningfully affect absorption, which is the main reason patients with active social schedules prefer tadalafil over sildenafil.

What to expect

Onset around 30 minutes for on-demand; daily-dose patients have steady coverage from day 4–5 onward. Common side effects: back pain, muscle aches (specific to tadalafil among PDE5s), headache, flushing. Most are mild and dose-related.

Daily tadalafil vs. on-demand — which is right?
Daily is for patients who want steady coverage and don't want to plan dosing around activity, or who also have BPH symptoms. On-demand is for patients who'd rather not take a daily medication. Both are clinically equivalent for ED itself; the choice is lifestyle.
Why the back pain side effect?
Tadalafil has secondary activity on PDE11, which is present in skeletal muscle. The back/muscle aches some patients report are likely from that off-target inhibition. Sildenafil is more PDE5-selective, so patients who can't tolerate tadalafil often switch to it.
Can tadalafil treat my prostate symptoms?
FDA-approved for BPH at the 5mg daily dose. Improves urinary flow, reduces frequency, and reduces urgency in many patients. Doesn't shrink the prostate the way finasteride does — different mechanism — but the symptomatic overlap with ED treatment is one of the cleaner two-in-one wins in men's health.
