PT-141 — the desire peptide.
PT-141 (bremelanotide) is the FDA-approved on-demand peptide that acts on melanocortin receptors in the brain to drive sexual desire. Works on the wanting, not the plumbing — making it complementary to PDE5 inhibitors, not a replacement.
What it is
PT-141 is the synthetic peptide bremelanotide. Sold under the brand name Vyleesi for hypoactive sexual desire disorder (HSDD) in pre-menopausal women, and used off-label in men and post- menopausal women for similar indications. Unlike sildenafil or tadalafil — which act on blood-vessel smooth muscle to enable erection — PT-141 acts centrally on the brain.
Mechanistically: it's a melanocortin receptor agonist (primarily MC3R and MC4R). Those receptors sit in brain regions governing sexual arousal — particularly the hypothalamus. PT-141 turns up the wanting signal, where ED medications turn up the can.
How it works
Activation of MC4R in the paraventricular nucleus of the hypothalamus drives sexual arousal pathways in both sexes. PT- 141 binds these receptors selectively, producing a transient increase in sexual desire that's independent of vascular response.
Onset is typically 30–60 minutes after injection. Duration is 1–4 hours. It works only when the receptor signal lines up with the patient's actual context — meaning it doesn't produce desire out of nowhere, but it amplifies desire in patients whose baseline drive has dropped.
Who it's for
PT-141 tends to fit:
- Women with low desire (HSDD), particularly pre-menopausal
- Men whose ED is partly desire-based, not just vascular
- Patients who've found PDE5 inhibitors insufficient because the issue is in the wanting rather than the function
- Post-menopausal women whose hormone replacement isn't fully restoring libido
PT-141 is on-demand, not a daily protocol. Patients use it situationally, not continuously.
Dosing and cadence
Typical clinical use: 1–2mg subcutaneous injection, 30–60 minutes before anticipated activity. The branded Vyleesi auto-injector is 1.75mg fixed-dose. Compounded vials let us titrate dose to individual response.
Most patients respond at 1mg. Higher doses produce more nausea (the primary side effect) without proportional benefit. We start conservative and adjust.
What to expect
Effect onset 30–60 minutes after injection. Duration of useful window 1–4 hours. Common side effects: transient nausea (the main one), flushing, occasional headache. Less common: temporary skin darkening with frequent use — usually a non-issue at on-demand cadence but worth flagging.

- No — they treat different problems. PDE5 inhibitors (sildenafil, tadalafil) act on the vascular side and help with the mechanical ability. PT-141 acts centrally and helps with the desire side. Patients with both desire and function complaints often use them together.
